Ljubljana 28 September 2017 – Government Office for Development and European Cohesion Policy, the Managing Authority for ERDF, ESF and Cohesion Fund, issued a grant decision for the project Model of community-based long-term care. The project which is worth EUR 2 million is set to receive the ESF contribution equalling EUR 1.6 million.
The aim of the project is to develop and implement a standardised training curriculum for the staff working at the single entry point and deliver interdisciplinary training both for long-term care providers and informal caregivers and volunteers in selected pilot areas. The project will be complemented by monitoring and evaluation of the pilot projects and coordination of activities between pilot areas and the Health Insurance Institute of Slovenia which will be in charge of setting up the IT support. The result of pilot testing of approaches to better integrate long-term care services will be the setup and operation of a single entry point within the long-term care system. Project activities will thus importantly contribute to enhancing social inclusion and reducing the risk of poverty, while fostering the provision of affordable, sustainable and high quality services, including healthcare and social care services.
